The main goal of this project is to provide more autonomy to state health institutions, allowing them to manage finances and personnel more effectively, as well as to implement modern management methods, develop paid services, and optimize the use of budgetary funds.
According to the justification note, state medical institutions currently face a number of systemic problems, including:
- insufficient financial and managerial independence, especially in the area of personnel;
- limited opportunities for attracting additional resources and implementing modern management practices.
As part of the pilot, it is planned to introduce temporary rules for regulating the activities of state medical institutions. This implies the creation of a single fund for consolidating financial resources, with subsequent redistribution according to priority areas. In addition, institutions will have the right to independently determine their organizational structure and staff size based on the number of patients, the range of services, and technical equipment.
This will allow medical organizations to better adapt their activities to the needs of the population and improve the quality of medical services.Particular attention will also be paid to financial autonomy. In particular, it is planned to allow health institutions to retain unused budget balances at the end of the financial year, which should contribute to optimizing expenses and creating reserve funds for modernization and the purchase of high-tech equipment.
Furthermore, the project provides for the introduction of a new regulation on labor remuneration and financial incentives. This regulation will preserve the guaranteed rights of employees but will propose to abandon the formal approach to salaries based solely on position or rate. The new remuneration system will take into account work performance and enhance the significance of the labor contract, giving managers the right to set salaries considering minimum guarantees.Regulation on remuneration and financial incentives for employees of health organizations in pilot mode.
It is important to note that the activities of state health institutions during the pilot will be carried out within the allocated funds for 2026, while the volume, quality, and accessibility of medical services under the State Guarantee Program will not be reduced.
Additional resources are planned to be attracted through donor and sponsorship organizations, for which medical institutions will be offered to open current accounts at OJSC "Eldik Bank".
One of the key innovations will be the development of pricing mechanisms for medical and other services. For organizations participating in the pilot, these mechanisms will not be subject to legislation on state and municipal services. The Ministry of Health, together with the Ministry of Economy and Commerce, has been tasked with developing a methodology for forming tariffs for paid services, taking into account benefits for socially vulnerable groups of the population.
The calculation and approval of prices for services will be carried out by the heads of organizations themselves according to the approved methodology. As stated in the document, this should contribute to creating a competitive environment, increasing financial revenues, and enhancing the motivation of medical workers.
The developers emphasize that the new methodology will not lead to unjustified price increases but will ensure fair pricing considering inflation, utility costs, and other factors. Currently, the cost of paid services in state medical institutions, according to the Ministry of Health, is 2-3 times lower than in private clinics.
The Ministry of Health is confident that the comprehensive implementation of the proposed measures will increase the interest of managers in work results, enhance the motivation of medical workers, and improve the quality of medical services, as well as patient satisfaction levels. An analysis of the results of the pilot will be conducted, and based on it, a report with recommendations for further reforming the healthcare system will be prepared.
