
As part of the new resolution, it is planned to implement a pilot regime in 20 state medical institutions covering various levels and regions of the country, with the mandatory preservation of the State Guarantee Program.
Main changes include:
Increased autonomy of medical institutions
Now, managers will be able to independently form the organizational structure and determine the staffing levels, taking into account the number of patients, the range of services, technical equipment, and performance indicators.

Medical institutions will be able to pool their financial resources into a single fund, redistribute funds according to priority areas, and retain unused balances for future equipment purchases and reorganizations.
New wage payment model
It is proposed to shift from the traditional payment system based on positions and rates to a results-oriented model. Managers will be authorized to set employee salary levels considering minimum guarantees.

Medical institutions are allowed to open current accounts at OJSC "Eldik Bank" for interaction with donors and sponsors.
Paid services and tariffs
The project also proposes the introduction of new pricing mechanisms for paid medical and related services within the pilot regime. These services will not be subject to legislation on state and municipal services.

- aim to maintain prices below market rates in the private sector.
Expected results
- According to the developers, the implementation of these measures will allow:
- to increase the motivation of managers and medical staff;
- to strengthen the financial stability of state medical institutions;
- to expand the range of services provided;
- to improve the quality of medical services and patient satisfaction.

In the event of positive results from the pilot, the mechanisms will be gradually implemented in all state medical institutions. Upon completion of the experiment, an analytical report will be prepared with recommendations for further reform of the healthcare system.
